CHEO, on behalf of the Kids Come First Health Team, is responsible for overseeing and delivering pediatric home care services across the Champlain region.
Many families of children and youth, including those with medically complex needs, asked for a more connected system of care. CHEO’s goal is to provide a more seamless and coordinated approach, so that it is easier for children and youth to get the right care, when and where they need it.

Pediatric home and community care services at CHEO
CHEO and partners of the Kids Come First Health Team work together to provide the following pediatric home and community care services:
- Care Coordination
- Nursing: rapid response nursing, shift nursing at home and in school, and visit nursing
- Personal support services
- Physiotherapy
- Occupational therapy
- Nutritional services
- Medical supplies and equipment (where eligible)
- Information and/or referral to other health and social services in the community.
CHEO also manages the Family Managed Home Care (FMHC) Program, which provides an option for eligible families to receive funding directly to purchase home care services or employ care providers. Find more information about the Family Managed Home Care (FMHC) Program.

What if a physician or other health-care provider is making a referral on behalf of my child or youth? |
If your child or youth’s family physician, community pediatrician or other health-care provider has already made a referral, one of CHEO’s Care Coordinators from the Department of Integrated Care Delivery will call you when the referral has been received and reviewed for service eligibility. |
What happens next? |
The Care Coordinator will work with you to understand what service(s) your child needs and develop a care plan. |
New Inpatient Medicine Discharge Program at CHEO
The new Inpatient Medicine Discharge Program at CHEO provides extra support for families as their child transitions from hospital to home. CHEO’s community-based rapid response nurses are dedicated to ensuring a safe, smooth discharge experience by offering guidance and answering any questions families may have.
How we help:

We provide you with guidance, education and ongoing support.

We help you understand your child’s diagnosis, medications, medical equipment and discharge instructions.

We help you avoid emergency department visits and preventable hospital admissions.
What to expect: A nurse will call within one to two business days after your child’s discharge and continue to provide support until the program’s services are no longer needed.
Program hours: Monday to Friday, 8 a.m. to 4 p.m.
