Planning is underway to transfer pediatric home care services from Home and Community Care Support Services Champlain to CHEO, which is anticipated to take place in early August. This will be a step toward simplifying the pediatric health-care experience for families of children requiring care at home and in the community. Through consultations over many years, these families have asked for integrated, more connected care in acute and home care services.
Ensuring continuity of care is a top priority. We aim to make this as seamless as possible for families, service providers and staff. Services and contacts will remain the same. This anticipated transfer is a step in the plan approved by the Ministry of Health in December 2019. Ongoing planning will ensure a smooth transition with no disruption for patients and families.
Information has been shared with impacted stakeholders, including families and staff, and additional details will be provided as the planning process continues.
There is no impact to home care services for adults. Adult patients and families will continue to access home care services in the same way and through the same contacts at Home and Community Care Support Services Champlain.
