The Ministry of Health’s Family-Managed Home Care (FMHC) program provides an option for eligible families to receive funding directly to purchase home care services or employ care providers.
Parent(s)/guardian(s) are responsible for managing and overseeing their child’s care provider(s) and are responsible for various reporting requirements to CHEO.
How to access Family-Managed Home Care (FMHC)
As a first step, one of our Care Coordinators will complete an assessment with the child or youth to determine their needs and will work with parent(s)/guardian(s) to develop a care plan.
The Care Coordinator will also assess eligibility for the program, including the parent(s)/guardian(s) ability to take on the program responsibilities such as managing and coordinating care, and managing and reporting on funds.
